Yes! Pretty Cure 5: This one's weird. The girls have two completely different group dynamics, which form two distinct Five Man Bands. The left-hand FMB is normally seen when the group is acting as an ordinary group of True Companions and nothing more; when it's time to deal with matters more suited to a team of Magical Girls, like fighting the Monster of the Week, they switch to the right-hand FMB. (Well, except that Urara/Lemonade has a tendency to turn into a second Nozomi/Dream when the plot isn't focusing on either of them, putting her on the wrong side.)

The Chick Nozomi / The Hero Cure Dream
The Big Guy Rin / The Lancer Cure Rouge
The Hero Urara / The Chick Cure Lemonade
The Smart Guy Komachi / The Big Guy Cure Mint
The Lancer Karen / The Smart Guy Cure Aqua
The Mentor(s) Kouji and Natsu / Team Pet(s) Coco and Nuts

* Additionally, Milk is a Team Pet for both, and Kurumi/Milky Rose is a Sixth Ranger for both.

Yu-Gi-Oh! GX - The Loads and Loads of Characters never actually form any Five-Man Band, but the opening credits always show Judai posing with the characters who would fit the roles:

The Lancer 2 -- Manjyome, Sho
The Big Guy Hayato, then Kenzan
