The song is her dying dream, brought about by the fever, in which their roles are reversed, Desmond is a successful fishmonger, and the family lives in relative comfort. (The reversal of "Desmond" and "Molly" in the final verse is a clue to the true situation.) "Life goes on" in the chorus is a particularly cruel irony, since life will not go on for Molly.
