If Panem is supposed to be the USA, there are clear divisions already, with some regions more likely to rebel and others aren't. And it's quite unlikely, if not completely impossible, for a single city and its surrounding suburbs to vanquish the rest of the country. So, a large swath of the country didn't rebel, and therefore were not divided into districts. The rebellious regions were reconquered and organised into the thirteen districts. Those regions that did not rebel were allowed to keep their original status, and are not required to send two tributes to the Hunger Games every year.
