* The two above things can be explained by the fact that the versions of the Doctor only remember meeting other versions of the Doctor ''when they're with other versions of the Doctor''. That's why Nine didn't remember he'd turn into Ten despite having met Ten as Five, but it's why Ten could remember Five seeing what Ten did once Five and Ten were together. When he's on his own, the Doctor probably has a general idea that he's met other versions of himself, but it's only when he's ''with'' those other versions that he can recall details. It's why Eleven was still uncertain about whether he'd saved Gallifrey even though he was essentially told as much... once he wasn't around the Doctor that told him so, his memories got muddy.
