Unfortunately we had to reject your second try for two reasons: one, you seem to have deleted all the categories and bottom-of-page markup from the page template -- this is a critical part of integrating the page functionally with the wiki and needs to be there. Second, you created the page with double quotes around its title. The latter wouldn't normally be a reason to bounce the page -- we would have just renamed it right after approval and dropped you a note about it -- but since you're going to have to recreate the page anyway, we wanted to let you know to name it right.
Also, something went wrong with your uploaded file "Book Cover .png" -- the file wasn't there to be approved. I'm not sure why. You'll need to re-upload it. When you do, we strongly suggest you give it a somewhat more descriptive name. There is more than one book in the world, and some of them have multiple covers.
