Lost Judgment/Nightmare Fuel

On top of the intense display of maggots and desiccated flesh of the corpse, Mokoshiba's body as discovered in the intro has to be one of the sickeningly detailed rotting corpses ever depicted in a video game. The camera makes sure to even slow down so we can get a long look at the horror of the decay and physical abuse.
The general theme of the game is how terrifyingly lethal bullying can be. We are shown multiple examples of it driving the victims to not only suicide, but also depression, self-harm, and various other forms of self-harm.
The two Big Bads of the game both have their own flavor of terror:
Yu Kitakaka/Kuwana, while his motives are rooted in sympathy, he's also a terrifyingly effective and remorseless Serial Killer who devoted his life to killing those who bully others in the most extreme manner possible. While we are left to guess at some of the horror he has inflicted, he's shown to helped cause the brutal torture and murder of at least one person and perverting two other law-abiding people into becoming killers themselves.
Hidemi Bando, especially via his proxies Kazuki Soma and RK, is one evil SOB despite being a Non-Action Big Bad. While he never gets his hands dirty personally, he's shown to have the ability to pervert the justice system and destroy evidence at will so he can keep running a shadow army of criminals to do his dirty work. He's even willing to do this even when it's clear his frail justification of keeping evil on a leash falls apart and he knows he's essentially allowing his own country to be terrorized and corrupted, so long as he gets what he wants.
- His right hand man is his own bundle of terror. Kazuki Soma is a sociopath Serial Killer who will murder anyone and legitimately enjoys it. He doesn't hesitate to order sadistic torture, brutally murders Yoko Sawa in cold blood just because he could, to the horror of both Yagami and Kuwana, and very nearly becomes a Karma Houdini, which is only barely averted, else he'd be walking free to continue spilling blood.
Both Ehara and Reiko Kusumoto are horrifying if you consider what they became. Both were formerly law-abiding parents (with the former even being a policeman) who became willing participants in criminal conspiracy, torture, and murder. The worst part of all is they were driven to these depraved extremes because the justice system was at best toothless and at worst hopelessly corrupted to prevent either from getting proper justice for their loved ones.
Kuwana's hidden video of the bullying mentioned in the backstory. Several teenagers with no conscience are shown to have sadistically beaten and tortured a young man to the point of trying to kill himself. The sickest part is you can see the poor boy begging for mercy yet he gets none while his tormentors are free to beat and humiliate him.
- The ringleader Shinya Kawai is a scumbag of the worst sort. Not only did he walk after being a vile, unrepentant violent sadist to a minor, he's shown to have grown up to be an even more vile piece of human garbage by becoming a human trafficker for the yakuza, and even brags about his earlier sadism. No wonder Reiko Kusomoto had him murdered in cold blood.
