Please check All The Tropes:Uploading and Adding an Image to a Page for all the details, but basically, we are required by both law and the Creative Commons license we use to have the license under which we may use the image. I suspect that it's going to be "fair use" (which you can put on the page by editing it and adding the {{fairuse}} template to it) but in case it's not please check the help page for details.
Not strictly required but strongly encouraged is the URL where you found the image. Twice in ATT's life we've suffered catastrophic damage that resulted in lost files, and we still haven't restored some because people don't remember where they came from or even what they looked like. Putting the source URL in the image description will let us find it and restore it if that ever happens again.
Categories identifying and classifying the image are also encouraged.
